USA, Report: Serious Safety Failures in Titan Submersible Procedures

Portland (Maine, USA), Aug. 5 (LaPresse/AP) – The U.S. Coast Guard’s report on the 2023 implosion of the Titan submersible off the coast of Canada found that the safety procedures of OceanGate—the company that organized private underwater expeditions to view the Titanic wreck—were “severely lacking,” emphasizing that the core of the company’s failures lay in the “glaring disparities” between official safety protocols and actual practices. Investigators found that the design, certification, maintenance, and inspection process of the submersible were all inadequate. In the report, which spans over 300 pages, investigators repeatedly highlight OceanGate’s practice of downplaying, ignoring, and even falsifying key safety information to boost its reputation and evade regulatory scrutiny. OceanGate ignored “red flags.”
